Position Description
Job Title: Registered Nurse (RN) - Bilingual in Spanish or Chinese
Location: Northern Queens, New York (Fieldwork Required)
Overview:
A nationally recognized evidence-based nurse home-visiting program, is seeking a Nurse to join their team. This program serves high-risk, low-income, first-time mothers and their families, providing critical care and support. The Nurse Home Visitor will work with pregnant women and infants throughout Northern Queens, assessing medical and social needs, following prescribed care plans, and monitoring outcomes.
The Nurse Home Visitor will report to the Nurse Supervisor and be responsible for delivering home visits, education, and case management services to women from pregnancy through their child’s second birthday.
Salary: $90,000 annually

Key Responsibilities:
- Home Visitation & Case Management:
- Provide home visits to pregnant/parenting women and their families in the Northern Queens area.
- Educate, support, and manage cases for women from pregnancy to the child’s second birthday.
- Carry a caseload of 25 clients, with a mix of home visits and team support.
- Clinical Duties:
- Build therapeutic relationships with families using motivational interviewing and reflective listening.
- Assess physical, emotional, social, and environmental needs of women and families, adhering to guidelines.
- Establish client goals and support families in achieving them through education, resources, and referrals.
- Collaborate with other professionals to coordinate services and ensure proper follow-up.
- Complete documentation for Medicaid billing and other required reports.
- Perform related duties as needed to support program goals.
Qualifications:
- Required:
- Bachelor’s Degree in Nursing (BSN) required (or currently enrolled in a BSN program).
- Valid New York State RN license in good standing.
- Bilingual in Spanish OR Chinese
- Strong written and verbal communication skills.
- Basic computer proficiency.
- Must have a valid driver’s license and insured automobile.
Preferred:
- Experience in maternity or labor and delivery nursing.
- Home visiting or fieldwork experience.
Work Schedule:
- Full-time (35 hours/week)
- Monday to Friday, 9:00 AM to 5:00 PM, with flexibility as needed
- Training duration: 6-12 months, with a combination of live virtual sessions and in-person shadow visits
- Travel required for fieldwork, including a car for client visits
