Position Description

Job Overview – Intake RN (MLTC):

Compensation: $95,000 – $110,000/year

Location: Bronx, NY

Schedule: Monday to Friday, 9:00 AM to 5:00 PM

Join our client’s care coordination team as an Intake RN (MLTC) in the Bronx, NY, conducting UAS-NY assessments and evaluating eligibility for Managed Long-Term Care services. In this community-based role, you’ll support new member outreach, assess clinical and safety needs, and coordinate with interdisciplinary teams to ensure timely enrollments. Ideal candidates are proactive RNs with strong assessment skills and experience in acute care, home care, or MLTC settings.

Responsibilities as the Intake RN (MLTC):

  • Community Outreach & Assessment: Conduct home and field visits to evaluate eligibility, safety, and clinical appropriateness for MLTC enrollment.
  • UAS-NY & Documentation: Complete UAS-NY assessments and maintain accurate, timely records in compliance with DOH and MLTC guidelines.
  • Care Coordination: Work with Intake, Outreach, and clinical teams to initiate services and support new member onboarding.
  • Communication & Education: Liaise with potential members, families, referral sources, and physicians to support smooth transitions.
  • Referral & Outreach Support: Follow up on referrals, leads, and outreach efforts to drive enrollment and community engagement.

Qualifications for the Intake RN (MLTC):

  • Education: Bachelor’s degree in Nursing (BSN) is required.
  • Licensure: Current New York State Registered Nurse (RN) license is required.
  • Experience: Minimum 1 year of experience in an acute care setting required, with experience in home care or care management preferred.
  • Technical Skills: Proficiency with UAS-NY assessments and Microsoft Office Suite is required.
  • Skills & Attributes: Effective communication and interpersonal skills with a self-motivated, professional demeanor and ability to manage time efficiently in the field.
  • Language Requirement: Bilingual in Spanish or Chinese highly preferred.
