Join PACE CNY as a Clinic RN!

The Clinic RN at PACE ( 115 Creek Cir, East Syracuse, NY 13057 ) works in a clinic/practice setting in collaboration with our talented interdisciplinary team and provides direct nursing care to PACE CNY participants. The RN is responsible for assessment of health needs; promotes and aids in maintaining the health of the participants through teaching and initiating appropriate preventative interventions; and acts as the primary support to the providers.

Schedule: Monday - Friday 8:00am - 4:30pm

Pay: $38.51 - $43.25 per hour

Responsibilities

  • Conducts nursing assessment of participants and provides nursing care for ongoing or episodic illnesses.
  • Reviews participant’s medical record to ascertain pertinent information for follow-up during clinic visit.
  • Observes participant and assesses for changes in clinical and/or functional status and initiates appropriate follow-up actions; reports changes in condition and/or concerns to provider or clinical coordinator as appropriate.
  • Provides skilled nursing care and prescribed treatments to PACE participants; assesses and documents data relevant to the participant’s current health status, medical history and care plan needs; communicates changes in needs to appropriate team members.
  • Provides telephone triage; makes an assessment of the participant’s problem after careful questioning and active listening to what is said and through subtle indicators such as incomplete responses, tone of voice, etc.; provides advice for minor problems; requests Home Care skilled nursing visit for further assessment or makes arrangements for the participant to be seen in the clinic the same day/next day as appropriate; seeks guidance from a provider based upon participant’s symptomology, risk factors, advance directives, etc.; facilitates 911 activation for severe, life threatening symptoms or caller’s preference.
  • Prepares, administers and records medications, treatments and procedures according to clinic policies, procedures and standards of nursing practice. Transcribes provider orders; implements or communicates orders to appropriate staff for follow-up and implementation (congregate housing staff, home care, etc.), processes encounter paper work; maintains clinic logs and tracking ledgers as required; may assist with new employee orientation. Provides 1:1 education to participant and/or caregiver/family as appropriate for changes in medications/initiation of new treatment, new diagnosis, etc. and documents intervention in EMP.
  • Assists in patient care coordination; promotes and maintains health of participants enrolled in PACE through health teaching, counseling, and initiation of appropriate preventative interventions; maintains patient confidentiality; promotes an environment for participants that acknowledges and respects the participant’s individuality and rights during treatments and procedures.
  • Recognizes and responds to emergent situations in the Day Center and manages staff and others as necessary.
  • Ensures information and reports are communicated from reporting and care planning meetings to clinic and staff daily.
  • Provides for the emotional, physical comfort and safety of the participant/family members while in the clinic; may also provide direction and guidance to other clinic staff to meet participant’s needs.
  • Orders and maintains stock supplies and medications as assigned.
  • Participates in QAPI activities and attends assigned committee/task force meetings as requested.
  • Demonstrates adherence to all compliance policies and procedures. Is responsible for promoting and fostering compliance in the workplace.
  • Ensures the highest quality of care by performing responsibilities according to the highest professional standards.
  • Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and can verbalize their role during an event.

Qualifications

  • Degree from an accredited school of nursing required.
  • Must have NYS RN license in good standing and current registration.
  • Minimum one (1) year of acute care/long term care or physician office setting preferably in geriatric setting required.
  • Proven patient assessment skills.
  • Current knowledge of best practice nursing standards and care.
  • Demonstrated organizational skills and the ability to prioritize.
  • Strong clinical and multitasking skills.
  • Excellent interpersonal skills and the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
  • Prior charge nurse experience a plus.
  • Proficient with computers and able to learn new software rapidly.
