The Hebrew Home at Riverdale by RiverSpring Living, a world-renowned long-term care organization located in Riverdale – North Bronx, serves over 18,000 people every day, both in the greater New York community and on our beautiful 32-acre overlooking the Hudson River.
RiverSpring Living provides a full range of care solutions, including nursing home care, managed long-term care, assisted living programs, senior housing, rehabilitation services, and specialized services such as elder abuse prevention and memory care.
It is the responsibility of the Admissions Nurse to participate in the process that assures the clinical, psychosocial, and physical needs of the elderly are met through making appropriate assessments and recommendations. Additionally, the Admissions Nurse makes referrals for placement within the Hebrew Home continuum of care.

It is the responsibility of the Admissions Nurse to participate in the process that assures the clinical, psychosocial, and physical needs of the elderly are met through making appropriate assessments and recommendations. Additionally, the Admissions Nurse makes referrals for placement within the Hebrew Home continuum of care.
Key Responsibilities
- Complete clinical assessments and make recommendations to support appropriate placement within our facility.
- Review and analyze medical records and electronic referral systems (Allscripts, CarePort, faxed referrals) for accuracy and completeness.
- Communicate with referral sources, hospital discharge planners, patients, and families.
- Coordinate with social workers and other staff to support timely and appropriate admissions.
- Verify insurance, obtain authorizations, and initiate insurance cases as needed.
- Serve as a knowledgeable resource on Sub-Acute and Long-Term Care programs within the Hebrew Home
- New York State RN license and 3 years of clinical nursing experience, preferably in geriatric care or LPN with at least 5 years of documented geriatric care experience.
- Experience in a skilled nursing facility is strongly preferred. Strong assessment skills; critical care experience a plus.
- Excellent communication (written and verbal), time management, and organizational skills.
- Ability to interpret clinical and psychosocial needs and align them with the appropriate level of care.
- Proficient in Microsoft Office and electronic referral platforms. Service-minded and committed to ethical, policy-driven practices aligned with NYS DOH regulations
- Annual salary: $100,000 - $108,000
