Position Overview

I. SCOPE OF ROLE:

ACT is an intensive, community-based multidisciplinary treatment team that provides person-centered care

to a shared caseload of adults whose needs are not able to be met through traditional outpatient services. In

collaboration with a psychiatric care provider, the team nurses assist with medication management and

administration, through documentation as well as monitoring clients for medical problems or side effects.

Nurses will communicate and coordinate services with other medical providers, engage in health

promotion, prevention, and educational activities. The nurse will educate other team members to help them

monitor psychiatric symptoms and medication side effect and work with clients to develop strategies to

maximize the taking of medications as prescribed.

II. ESSENTIAL DUTIES & RESPONSIBILITIES:

Clinical Skills

 Provide symptom and medication assessment, evaluation, education, and management to individual

with persistent mental illness. This includes consultation with Psychiatrist/PNP, hospital,

community providers, client, and client’s family/guardian for care.

 Administer both intramuscular and supply medications to consumers as directed by Psychiatrist or

Nurse Practitioner

 Assess the physical health needs of participants and make the appropriate referrals to community

based medical care.

 Ensure pertinent medication related information is conveyed to hospital staff in the event of client

hospitalization.

 Provide education on physical health to ACT consumers through individual services and groups as

needed.

 Provide training to program staff on disease, treatment options, and new advances in disease

management.

 Identify and include the client’s support network and applicable involved community partners in

care coordination.

 Assist clients in identifying and accessing resources and collaborate with community resources on

behalf of clients.

 Assist clients in the development of healthy coping mechanisms in individual or group settings.

Data and Accountability

 Conduct baseline health assessments and biannual physical health assessments for all ACT

consumers thereafter.

 Manage the Medication Room, MAR Book, and other medical documentation.

 Maintain client records and all other required documentation, such as progress notes, in a timely

and confidential manner in accordance with agency policies and procedures

Job Description

 Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and

stabilization services as needed, including facilitating access to inpatient facilities for behavioral

health care.

 Provides 80 percent of treatment contacts in the community, including leading groups when

clinically indicated.

 Ensures all documentations are completed in a timely manner and remain in compliance with

agency and OMH regulations.

 Coordinate services with other specialist on the ACT Team, i.e. Psychiatrist, Substance Use

Specialist, Employment Specialist, Nurse.

Compliance

 Represent the agency and its services to the community in a positive and professional manner.

 Participate in required staff meetings, including regular supervision and daily clinical staff

meetings.

 Participate in any required training, consultation calls, and onsite visits or review when required by

contract.

 Establish and maintain collaborative working relationships with; clinical, administrative, support

staff, community partners, clients, and family/natural supports.

 Perform S:US charting requirements, correspondence, and other paperwork in a complete and

timely manner.

 Communicate pertinent consumer and operational information to the Team Leader and Senior

management

 Assists in the planning, developing and implementation of goals and training to further develop

participants skills.

 Responsible for reporting and documenting all incidents to Team Leader within 24 hours.

Operations

 Process and sign medication orders, complete medication refills, and prior authorization for

specific medications based on provider request.

 Ensure access to medications through timely refills, by obtaining sample medication per order and

by appropriate use of discretionary funds or patient assistance programs for medications.

 Accept delegation and follow through with specific expanded assignments and responsibilities

within S:US system such as quality improvement teams, task groups, committees, consultations,

mentoring or training responsibilities.

 Demonstrate teamwork and goal-oriented productivity.

 Operate as a member of multidisciplinary team and ensure services are provided in the community

and clinical setting in required by contract.

 Other duties as assigned.

Qualifications

III. REQUIREMENTS

Required Education And Experience

 State of New York Registered Nurse Licensure

 CPR/First Aid Certification

 Ability to communicate and engage effectively with individuals of varied backgrounds, trauma

experiences, cultures, education level, and socio-economic status.

 Must possess excellent communication skills, including the ability to send and receive oral and written

messages, in a clear, positive, prompt, and appropriate manner, while maintaining customer

confidentiality.

Preferred Education And Experience

 One year of experience working with adults with severe mental illness

 Bilingual in any language a plus not required.
