Scholarship Application

 

Eligibility Criteria:

o Applicant must be a member of the AORN NYC #3304 Chapter and show interest in the field of perioperative nursing
o Applicant must be in an accredited program
o Eligible are nursing students in a program leading to initial licensure as an RN, baccalaureate, masters and doctoral candidates

 

Scholarship Amount: AORN NYC 3304 Chapter will award 4 scholarships in the amount of $1,000 each. Scholarships will be awarded at the December Chapter Meeting. The scholarship will be paid upon submission of an official transcript after successful completion of the fall semester.

 

Selection Criteria – All applicants must submit:

o A typed, double-spaced 250-word personal statement addressing your interest in perioperative nursing and professional goals.
o Academic Transcript (official transcript is required only if selected)
o Proof of acceptance in an accredited nursing program
o Accurate completion of the scholarship application
o Copy of government issued photo identification (drivers license, passport)
o Resume
