Scholarship Application

Eligibility Criteria:
o Applicant must be an active participating member of the AORN NYC #3304 Chapter and show interest in the field of perioperative nursing
o Applicant must be in an accredited program
o Eligible are nursing students in a program leading to initial licensure as an RN, baccalaureate, master and doctoral candidates

Scholarship Amount: AORN NYC 3304 Chapter will award 3 scholarships in the amount of $1,000 each. Scholarships will be awarded at the December Chapter Meeting. The scholarship will be paid upon submission of an official transcript after successful completion of the fall semester.

Selection Criteria – All applicants must submit:
o A typed, double-spaced 250-word personal statement addressing your interest in perioperative nursing and professional goals.
o Academic Transcript (official transcript is required only if selected)
o Proof of acceptance in an accredited nursing program
o Accurate completion of the scholarship application
o Copy of government issued photo identification (drivers license, passport)
o Resume
o Two letters of endorsement (Suggested are academic advisor, instructor, employer, peer)

Expectation of Scholarship Winners:
o Attend at least 4 chapter meetings within the calendar year upon receipt of scholarship
o Actively participate in AORN NYC Chapter meeting or sponsored event
o Update AORN NYC board with your academic progress 1 year after receipt of scholarship
